No, actually when it comes to medical problems, the questionj becomes whether you are expected to recover or not. If you are expected to recover, then you are supposed to fast at another time, if you are not then the payment you mention is suggested. OTOH, considering that the majority of Turks do nto fast throughout the entire month or Ramadan, and that only a fraction of those wouild ever consider paying any such monies, you are in an extreme majority, so I am sure your cleaning lady considers herself extremely lucky..
Actually, I know many people who cannot fast who do pay. But then, I suspect we are in different circles.
That is obvious on so may levels. Despite that however, my numbers are macro, you seem to look at things froma micro level.
In any case, if your friends pay money because they are ill from something they expect to recover from then they are simply wrong in paying rather than fasting at a later date.
I tried fasting once, but my body is just not built for speed, so I went back to slowing.
